    Jan Sweat

    Vice president of Learning Solutions, Jan Sweat comes to CHAP with over 25 years of experience in healthcare, with a focus in the post-acute industry. For nearly two decades, she worked for Kindred at Home in sales, sales leadership, operations, and talent development.   

    Prior to joining CHAP, Sweat directed the education and success of tens of thousands of healthcare professionals in Hospice, Home Health, and Palliative Care nationwide. In addition to her classroom impact, she spent several years as a growth strategist connecting corporate strategy to field implementation and interim area management. These roles equipped her with a unique perspective to solve provider problems with robust solutions.    

    After graduating from North Georgia College and State University earning a degree in Sociology with a minor in Psychology, she served as a case manager in a metro Atlanta hospital and a nursing home social service director for several years, where she supported countless patients and families.

    Daniel Stephens found his purpose in helping community-based healthcare advocates increase their impact and ability to help more patients receive care. Daniel is passionate about helping healthcare leaders grow their businesses and advance their mission through strategy, innovation, and efficiency.

    He specializes in business development leadership and sales experience in the healthcare industry with a focus on DME/respiratory, home infusion, home health, hospice, and palliative care.     Keri comes to CHAP with 17 years’ experience in the post-acute healthcare industry and has held a variety of positions from direct patient care as a Registered Nurse to chief executive in both home health and hospice organizations. She has a passion for community-based care and enjoys educating others and contributing to their growth and development as a healthcare professional. Keri previously served as a board member/officer for the Oregon Hospice & Palliative Care Association. She received her Master’s in Business Administration from the University of Texas at Tyler, and her Nursing degree from the University of Wyoming.

    Most recently, Keri served as a Home Care Surveyor for 5 years with The Joint Commission where she conducted surveys for home care organizations across the country and provided education to organizations regarding standards, regulations, and operational processes. During her last year and a half as a surveyor, Keri was responsible for the onboarding and orientation process of new home care surveyors. She has led several projects and workgroups to include educational presentation development, technological survey advancements, standard operating procedure development, and new hire orientation development. In addition to developing educational presentations, Keri has been a speaker/presenter at state association conferences across the country.

    Formerly, Keri served as Executive Director for Coastal Home Health & Hospice where she was responsible for overall agency operations and management. She also served as a Director of Accreditation for CHAP responsible for operational oversight and survey activities to validate consistent application of accreditation and regulatory standards for community health providers with a focus on safe, quality patient care delivery.

    CHAP is happy to offer our Home Care customers an Accreditation Intensive. This is a great training for all of our Home Care (Private Duty) customers regardless of what stage of the accreditation process you are in. This workshop also provides the opportunity for consultants to become Home Care Certified. This training will help prepare you to work with customers and guide them through the process of becoming accredited. A post workshop exam is required to become certified (passing score of 80% is required).

    This session is intended for all CHAP Home Care accredited agencies or those who wish to learn about the CHAP Standards of Excellence for Home Care. The two-day session will provide an overview of the CHAP standards for Home Care, the most frequent citations in each area, and tips for dealing with these challenges. This service line was previously known as private duty and includes both skilled and non-skilled private duty services.

    This event is for all of our customers regardless of what stage of the accreditation process you are in. This is an interactive, virtual event designed to promote the active participation of each individual attendee and, as such, each registration is an individual registration.

    Jennifer Kennedy

    Vice President of Quality, Standards, and Compliance

    CHAP - Community Health Accreditation Partner

    Dr. Jennifer Kennedy is the Vice President of Quality, Standards, & Compliance. She spent many years as a leader and nurse in diverse healthcare settings with the past 25 years in hospice and palliative care.

    Dr. Kennedy came to CHAP in 2021 with a vision of moving the organization’s quality forward as “the” accreditation partner of choice for the majority of community-based providers. She believes no matter what type of care a patient receives or how many times they receive care, every experience should be of the highest quality.

    Kennedy leads the CHAP quality team and interacts with CMS related to regulatory and quality issues. Kennedy serves as faculty at national and state conferences, is widely published in medical trade journals, contributes to and reviews health care published materials, is a published author, and teaches graduate healthcare administration courses at the university level.

    Kennedy serves as a member of the Hospice & Palliative Care Network of Maryland board of directors and has been honored by the UK’s St. Christopher’s Hospice as a Nursing Pioneer for their 2020 Palliative Care Nursing Project.

    Dr. Kennedy has a Doctoral Degree in Health Care Education and Policy, a dual master’s degree in health education and Case Management, a bachelor’s degree in nursing, and is a Certified Healthcare Compliance Professional.

    Bill Taylor

    CPA

    Bill Taylor is a North Carolina based CPA who learned Hospice and Home Health at MVI where he led the consulting division for many years. He started his professional life as Journalism major at the University of Tennessee and later gravitated toward accounting after running a newspaper plant for several years. Semi-retired, Bill now does the occasional project for MVI, processes Medicare Cost Reports and teaches for CHAP.

    Bill also serves on the NHPCO Finance and IT Steering Committee.



    Denise Stanford, MS, SHRM-CP

    Denise Stanford is passionate about the impact of education and training on quality improvement in healthcare. As someone who has utilized community-based healthcare services for loved ones, she understands the importance of high-quality care and services.

    Denise personally experienced how education and training can transform an individual’s life and the lives of those they touch. Denise has been working as an educator in community-based healthcare for nearly a decade. She greatly enjoys creating learning experiences that engage learners and teach real-time skills that can be immediately applied on the job. Her master’s degree in adult learning has allowed her to work on the development of workshops and e-learning for healthcare professionals, focus on training for community members, and serve as an adjunct professor working with first-time college students. In her role at CFE, she helps craft meaningful learning experiences and oversees the operation of the department.

    Vice president of Learning Solutions, Jan Sweat comes to CHAP with over 25 years of experience in healthcare, with a focus in the post-acute industry. For nearly two decades, she worked for Kindred at Home in sales, sales leadership, operations, and talent development.   

    Prior to joining CHAP, Sweat directed the education and success of tens of thousands of healthcare professionals in Hospice, Home Health, and Palliative Care nationwide. In addition to her classroom impact, she spent several years as a growth strategist connecting corporate strategy to field implementation and interim area management. These roles equipped her with a unique perspective to solve provider problems with robust solutions.    

    After graduating from North Georgia College and State University earning a degree in Sociology with a minor in Psychology, she served as a case manager in a metro Atlanta hospital and a nursing home social service director for several years, where she supported countless patients and families.

    Keri comes to CHAP with 17 years’ experience in the post-acute healthcare industry and has held a variety of positions from direct patient care as a Registered Nurse to chief executive in both home health and hospice organizations. She has a passion for community-based care and enjoys educating others and contributing to their growth and development as a healthcare professional. Keri previously served as a board member/officer for the Oregon Hospice & Palliative Care Association. She received her Master’s in Business Administration from the University of Texas at Tyler, and her Nursing degree from the University of Wyoming.

    Most recently, Keri served as a Home Care Surveyor for 5 years with The Joint Commission where she conducted surveys for home care organizations across the country and provided education to organizations regarding standards, regulations, and operational processes. During her last year and a half as a surveyor, Keri was responsible for the onboarding and orientation process of new home care surveyors. She has led several projects and workgroups to include educational presentation development, technological survey advancements, standard operating procedure development, and new hire orientation development. In addition to developing educational presentations, Keri has been a speaker/presenter at state association conferences across the country.

    Formerly, Keri served as Executive Director for Coastal Home Health & Hospice where she was responsible for overall agency operations and management. She also served as a Director of Accreditation for CHAP responsible for operational oversight and survey activities to validate consistent application of accreditation and regulatory standards for community health providers with a focus on safe, quality patient care delivery.

    This two-day workshop will focus on building up your operational toolkit with the goal of helping you effectively run (and GROW) your agency while providing client-centered care. This is a skill-based workshop with focused activities for the home care/private duty providers with a tailored approach to increase agency success. This workshop is open to anyone in the home care/private duty industry, whether already working in it, looking to get into it, or expanding your community-based services.

    Wendy is an accomplished and experienced nurse and operations leader, with over 25 years of dedicated service in the field of home care. As the owner of a successful healthcare consulting business, Wendy has demonstrated her expertise in various capacities, including as a CHAP certified consultant, legal nurse consultant, and accreditation surveyor.

    Throughout her career, Wendy has served in both state and national roles, notably as VP, Operations and Director of Advocacy for national home care agencies, where she played a pivotal role in shaping industry standards and advocating for the advancement of home care services. Her extensive experience extends to new business start-ups, where she has consistently proven her ability to navigate complex regulatory landscapes and establishing thriving ventures.

    In addition to her professional achievements, Wendy currently serves on the Recreational Therapy Licensing board, appointed by NC House Speaker, Tim Moore, and has served on national and state homecare and home health association boards, contributing her insights and expertise to further the goals of the industry. Denise has been working as an educator in community-based healthcare for nearly a decade. She greatly enjoys creating learning experiences that engage learners and teach real-time skills that can be immediately applied on the job.     CHAP Pediatric Care Certification is available to CHAP customers as part of the accreditation process. 

    The Pediatric Care Certification standards are based on peer-reviewed literature and integrate holistic clinical care principles of expert knowledge, assessment, and early identification of needs; patient-centered care and communication; care coordination; and the parent-provider relationship to elevate the goal of high-quality pediatric in-home care.

     

  • CHAP Compliance Program Standards (for all service lines).

    CHAP’s Compliance Program Standards are thoughtfully constructed and incorporate the U.S. Office of the Inspector General’s core program elements. We developed a new key performance area (KPA) named “Compliance Program” that will be effective June 1, 2024, in all CHAP service line Standards of Excellence and implemented in surveys on and after August 1, 2024.
