This event is for all of our customers regardless of what stage of the accreditation process you are in. This is an interactive, virtual event designed to promote the active participation of each individual attendee and, as such, each registration is an individual registration.

*This workshop also serves as new or renewing consultant training. There will be an additional registration fee of $49 for the consultant competency exam. It must be taken within 6 months of the training (and have 80% to pass) for CHAP Consultant Certification. This event has 11.25 CE for Nursing Professionals (or 18.5 for those attending both hospice and home health)

This nursing continuing professional development activity was approved by the Virginia Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Criteria for successful completion includes attendance of all scheduled sessions, participation in engagement activities, and completion of an evaluation. There are no relevant financial relationships with ineligible companies for those with the ability to control the content of this activity. 

Participating in this workshop in no way guarantees the outcome of the accreditation process. The content is meant to help participants understand the accreditation process, learn common areas of deficiencies, gain compliance with the most commonly cited standards, and utilize and implement the QAPI process.

Boasting an extensive 17-year background in the post-acute healthcare realm, Keri Culhane bridges the gap between direct patient care and high-level executive leadership. Her journey began as a Registered Nurse, evolving seamlessly into leadership roles within home health and hospice organizations.

Culhane recently served as a post-acute surveyor for 5 years, ensuring operational excellence across organizations nationwide. Furthermore, her commitment to educating others about standards and regulations is evident from her comprehensive role in onboarding and mentoring new surveyors.

Prior to becoming CHAP’s Clinical Nurse Educator, Culhane showcased her operational acumen as the Executive Director for a post-acute care agency. Additionally, her time as CHAP’s Director of Accreditation underscored her dedication to maintaining and ensuring the consistent application of regulatory standards, with patient-centered care, safety, and care quality at the forefront.

Armed with an MBA from the University of Texas at Tyler and a Nursing degree from the University of Wyoming, Culhane’s educational background solidifies her stature in the field.

Beyond her operational contributions, she’s an esteemed voice at state association conferences, sharing her insights and expertise.

Today, as CHAP’s Director of Education & Professional Development, she is not only poised to enhance community health initiatives but is fervently championing the cause of nurturing and shaping the future trailblazers of the healthcare realm.

Linda Hall is a dedicated healthcare professional with a passion for delivering innovative, high-quality care services. With over 25 years of experience in the post-acute care industry, Linda has worked extensively in adult and pediatric home health, hospice, and palliative care. Her deep expertise spans clinical operations, strategic planning, and the development of quality provider networks for health plans.

Linda’s career has been defined by her commitment to overcoming the challenges of delivering exceptional care in post-acute environments. She collaborates with organizations to enhance financial performance, operational efficiency, and clinical outcomes. Drawing on her in-depth understanding of day-to-day business operations, she guides healthcare providers toward achieving their strategic goals while prioritizing patient-centered care.

Linda is deeply involved in advancing care quality across the continuum of health services. She focuses on chronic disease management, medical home models, and end-of-life care to improve collaboration between providers and ensure better patient outcomes.

Linda’s passion for healthcare is both personal and professional. She has actively contributed to the evolution of healthcare by driving innovative approaches that enhance care processes and elevate patient experiences. A Registered Nurse with an MBA in Healthcare Management, Linda resides in Jupiter, Florida, where she remains dedicated to making a meaningful impact in the healthcare field.

Linda Lockhart brings decades of experience from the ground up, starting as a staff RN and progressing to administrative leadership, and expertise which spans both free standing and hospital-based home health agencies, providing a comprehensive understanding of the sector.

Her career trajectory includes a unique chapter with Health and Human Services in Florida, where she served as a fiscal intermediary, conducting rigorous weekly and monthly audits. This role forged her expertise in regulatory oversight and financial accountability. Driven by a commitment to quality, she pursued a graduate degree, aligning her expertise with her long-held goal of contributing to accreditation. Her work with CHAP, including nearly a decade as a Site Visitor, has enabled her to leverage this knowledge, guiding agencies toward achieving and maintaining the highest standards of care.

Review of CHAP Home Health Standards of Excellence will be conducted with the focus on those standards in each chapter that result in the most cited findings. This engaging workshop will include the use of a case studies that will be utilized to reflect areas in need of improvement and provide interactive learning. Nurse participants who complete all sessions, participate in engagement activities and submit the evaluation will be awarded 11.25 CE contact hours.
