As a chaplain and music therapist at Hospice of Louisville (KY), Joy received NCHPP’s National Heart of Hospice Psychosocial/Spiritual Care award. She is an author in Routledge’s acclaimed Series in Death, Dying, and Bereavement. As director of education and volunteers at Hosparus, Joy chaired NCHPP’s Research/Academic/Education section. As quality researcher, she redesigned integration of regulatory and accreditation standards with system-wide policies and procedures, and developed new technologies for efficient utilization. As director of education for the Hospice Education Network, Joy developed content and coached hospice and palliative care educators—nationally—on customized ways to improve their staff and volunteer education.

    Ms. Bramble is a senior human resources, talent management, and learning and development professional with 25-plus years of experience. She specializes in human capital strategy, development, alignment and change management. Formerly with National Committee for Quality Assurance, Ms. Bramble spearheaded a multi-faceted talent development initiative, focused on strength-based leadership development where she coached and mentored business professionals at all organizational levels. Prior to that, Ms. Bramble served at National Quality Forum as Senior Training and Development Manager where she delivered high-impact leadership development and corporate training programs and a variety of human resources functions.

    A certified Executive Coach, Ms. Bramble holds a BS in Business Management and an MA in Public Administration from Washington Adventist University. She lives in suburban Maryland with her husband. In her spare time, Ms. Bramble enjoys traveling and coaching and mentoring early-career professionals.

This on-demand series focuses on the Medicare Home Health conditions of participation (CoPs) effective January 13, 2018.  This series will provide up to date guidance CHAP has received from CMS including the CMS interpretive guidelines for the emergency preparedness regulations

    Barbara McCann

    CHAP President and CEO

    Barbara McCann is CHAPs President and CEO. She previously served as the Chief Industry Officer of Interim HealthCare Inc., the nation’s largest home care and hospice franchisor with over 300 offices in 44 states.  Ms. McCann joined Interim HealthCare in January 1998, and served as the Chief Clinical Officer overseeing the company’s corporate clinical operations team developing policies, procedures, and practice guidelines for the delivery of patient care, as well as compliance with federal law and regulation and professional standards of practice.  She also directed the national chronic care programs.  In 2010, Ms. McCann assumed the role of Chief Industry Officer acting as the company liaison and industry representative to Congress, CMS, MedPAC, the National Quality Forum, and the Patient-Centered Primary Care Collaboration, representing the roles of home care and hospice in medical homes and ACOs, as well other professional and consumer organizations.  In addition, Ms. McCann directs the national care transition programs partnering with hospitals, physicians, and payors to reduce avoidable readmissions and effectively transition chronically ill patients and their families to home self-management and safety. 

    Ms. McCann is a Phi Beta Kappa graduate of the University of California at Berkeley where she received her Bachelor’s degree in Social Welfare.  She received her Master’s degree from the University of New Mexico, and actively practiced as a medical social worker in a variety of health settings, including home care and hospice, prior to joining the JCAHO in 1981.

    Cordt Kassner, PhD

    CEO, Hospice Analytics

    Dr. Kassner is a hospice and end-of-life healthcare industry analyst. He incorporated Hospice Analytics in 2008 to merge Medicare datasets in unique ways, and with other national databases, to better understand and explain how healthcare is delivered to those near the end of life. He earned degrees in Social Work (BS, Illinois State University), Counseling (MA, Denver Conservative Baptist Seminary), and Health & Behavioral Sciences (PhD, University of Colorado-Denver). His prior work experience includes serving as CEO of the Colorado Center for Hospice & Palliative Care, and clinical and research responsibilities at the University of Colorado at Denver and Health Sciences Center.

    Cordt serves and has served on numerous local, state, and national boards and committees and has facilitated over 150 workshops and presentations. He served on three recent Medicare Technical Expert Panels for Hospice Quality, and is the Vice-Chair / Incoming Chair for the Community Health Accreditation Partner (CHAP) Board of Directors. Cordt has served on the faculty of the University of Colorado-Denver Health Sciences Center since 1992, and in 2012 was also invited to serve on the faculty at Duke University.

    Daniel Maison, MD, FAAHPM HMDC

    Seasons Hospice and Palliative Care/National Medical Director

    Dr. Maison has been practicing hospice and palliative medicine for nearly 20 years, working as a full-time hospice and palliative medicine physician since 2001.  He is currently a National Medical Director for Seasons Healthcare Management.

     

    He went to medical school at Georgetown University where he was elected to the Alpha Omega Alpha Honor society.  He trained and served as Chief Resident in Internal Medicine at California Pacific Medical Center.  He was named both Intern and Resident of the year while at California Pacific Medical Center.  He is Board Certified in both Internal Medicine and Hospice and Palliative Medicine.

     

    He is a nationally recognized consultant and speaker who has worked extensively at the local, regional, state and national level.  He has given key note addresses, chaired and conducted national workshops, and published articles on many issues relevant to Hospice and Palliative.  Other national educational activities include being a Master Facilitator for the Education for Providers of End of Life Care (EPEC), an organization that has trained thousands of healthcare providers in the provision of effective, excellent end of life care.  He has also served as a national mentor for new Hospice and Palliative Care physicians for the past 10 years.

     

    In July 2012, he chaired the preeminent national board review course for hospice and palliative physicians (with more than 1,000 physicians in attendance).  Most recently, he served on the Exam Committee for the National Hospice Medical Director Certification Examination and helped to create a national certification board exam for Hospice Medical Directors.

     

    Nationally, he has held several national leadership positions in organizations focusing on Hospice and Palliative Care.  Some of these activities include chairman of the Strategic and Coordinating Committee for Membership and Communities at the American Academy of Hospice and Palliative Medicine (AAHPM) and National Physician Section Leader for the National Council of Hospice and Palliative Professionals at the National Hospice and Palliative Care Organization (NHPCO).  He served on the board of directors for the Community Health Accreditation Program (CHAP).  And most recently, he a completed his as a Palliative Director for the Board of Directors for NHPCO.  He was most recently elected to the Advisory Board for Global Partners In Care.

     

    In 2006, Dr. Maison was elected a fellow of the American Academy of Hospice and Palliative Medicine and in 2013, he was nominated by the AAHPM as a Visionary in the fields of Hospice and Palliative Care.
