CHAP Core Standards of Excellence

Through the application of these standards, CHAP strives to help our customers:

The Standards of Excellence are built on four underlying principles:
- The organization's structure and function consistently supports its consumer-oriented mission.
- The organization consistently provides high-quality services and products.
- The organization has adequate human, financial and physical resources to accomplish its stated mission and purpose.
- The organization is positioned for long-term viability.
We continually review the Standards of Excellence to ensure they remain relevant for home and community-based health care providers.

CHAP Home Medical Equipment Standards of Excellence

CHAP Home Medical Equipment (HME) Standards of Excellence are deigned for Durable Medical Equipment, Prosthetics, Orthotics, and Supplies (DMEPOS) suppliers provide equipment, supplies and services to people to use in recovery, rehabilitation and/or management of chronic conditions. Their patients and clients depend on superior quality products and services, which are as varied as the patients they serve.
Such products and services include respiratory and sleep therapy equipment, complex rehabilitation supplies, tools for asthma treatment, continuous passive motion devices, hospital beds, external infusion pumps, wheelchairs and diabetic shoes, to name a few.
CHAP's standards for home medical equipment providers include teaching customers to set up and safely operate the equipment.
CHAP Pharmacy Standards of Excellence
CLICK TO DOWNLOAD. The CHAP Standards of Excellence are designed for an organization that provides pharmacy services directly to consumers in any setting, including ancillary or long-term care facilities.
CHAP Pharmacy Standards of Excellence are designed for pharmacy providers to recommend and dispense safe, reliable and effective medicine and treatments.
These professionals serve their communities in a variety of settings, including independent, community-based pharmacies; through mail order or online; or within hospitals and long-term care facilities.
Their services are invaluable within the community. These providers not only fill prescriptions, but also oversee medication management and responses for prescribed medications.
Pharmacies are integral to the health of a community. They need a partner that is likewise connected to home and community-based health care services.
CHAP Hospice Licensure Standards of Excellence v.1.1.1

CHAP is contracted with several states to complete the licensure site visit. This may enable providers to attain licensure sooner than through other means. This set of standards is a subset of the total Hospice standards and includes only those requirements which would need to be in place to obtain licensure.

The California Hospice Licensure Standards are a set of standards that include CHAP requirements related to aspects of licensure that are compliant with California requirements. as well as, California specific licensure requirements that are not addressed through CHAP hospice standards.
Infusion Therapy Nursing Standards of Excellence v.1.1.2
CLICK TO DOWNLOAD. The CHAP Standards of Excellence are designed for an organization that provides infusion nursing services directly to patients through a home health agency, ambulatory infusion center, infusion pharmacy or private duty nursing program.
CHAP Infusion Therapy Nursing Standards of Excellence are designed for providers to help patients receive critical medications without needing hospitalization. These nurses administer medication in homes, ambulatory care centers, nursing homes or through infusion pharmacy operations, providing support for patients of all ages.
These professionals also ensure better patient care and outcomes through ongoing monitoring and consistent follow-up visits. They need the most up-to-date standards and education to provide the care and quality patients need and expect.
Accreditation through CHAP helps ensure infusion therapy nurses stay abreast of the knowledge and technology they need to deliver the highest quality care throughout a community.

Palliative Care is the organized delivery of patient and family centered care that optimizes quality of life by anticipating, preventing and treating symptoms of individuals with serious illness.
Palliative care addresses the physical, emotional, social, and spiritual needs of the patient and the family, as well as facilitating access to information and choice. It is provided and coordinated by an
interdisciplinary team in any setting.
The CHAP (Community Health Accreditation Partner) Palliative Care standards are informed by providers of palliative care and the National Consensus Project for Quality Palliative Care.
