    This interactive course will help your agency be prepared for any emergency and stay compliant with CHAP Standards. 

    * For Hospice: The material covered in this course is specific to community-based hospice organizations and does not focus on the CMS Emergency Preparedness requirements for in-patient units/facilities. 

    Learning Objectives:

    • Verbalize understanding of the home health and hospice CMS Conditions of Participation and CHAP Standards as they relate to Emergency Preparedness. 
    • Verbalize understanding of the top six emergency preparedness tags and how to demonstrate compliance with the regulatory requirements.
    • Recognize common pitfalls and consequences that can result from a lack of emergency preparedness planning.
    • Identify recommendations for improving emergency preparedness within your organization.

    This course is approved for 1 Nursing Contact Hour

    Continuing Education: Community Health Accreditation Partner (CHAP) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation. To receive ANCC contact hours for this activity, participants must complete the entire course and complete a post-test with a score of 80% or higher.

    This one-hour course will help your agency be develop plans of care that are individualized and compliant!

    Developing a care plan that is individualized, compliant, and effective is a crucial component of successfully delivering care that is patient-centered with improved patient satisfaction and outcomes, whether those be curative or palliative in nature. This course will address what the Conditions of Participation and CHAP standards say about care planning, and the process for developing a patient-specific care plan, including a special focus on developing goals that are individualized, specific, and measurable.  

    Learning Objectives:

    • Verbalize understanding of the home health and hospice CMS Conditions of Participation and CHAP Standards as they relate to care planning. 
    • Verbalize understanding regarding how the home health and hospice interpretive guidelines define interventions, patient goals, and expected outcomes.
    • Demonstrate understanding of the care planning pathway and the differentiation between goals and interventions. 
    • Demonstrate the ability to develop individualized goals utilizing the S.M.A.R.T. acronym.

    Learners will gain the knowledge necessary to successfully complete OASIS-E assessments as evidenced by the ability to meet the following objectives. 

    • Demonstrate the ability to distinguish between accurately and inaccurately completed OASIS-E assessments in each section of OASIS-E.
    • Recognize common challenges associated with specific OASIS-E items. 
    • Identify resources available for use when questions arise during OASIS-E completion. 

    There are 18 learning modules totaling 5 CE credits.
