Charting the Course: A Guide to Effective Care Planning in Home Health & Hospice - (CHAP On-Demand)

Developing a care plan that is individualized, compliant, and effective is a crucial component of successfully delivering care that is patient-centered with improved patient satisfaction and outcomes, whether those be curative or palliative in nature. This course will address what the Conditions of Participation and CHAP standards say about care planning, and the process for developing a patient-specific care plan, including a special focus on developing goals that are individualized, specific, and measurable.  

Learning Objectives:

  • Verbalize understanding of the home health and hospice CMS Conditions of Participation and CHAP Standards as they relate to care planning. 
  • Verbalize understanding regarding how the home health and hospice interpretive guidelines define interventions, patient goals, and expected outcomes.
  • Demonstrate understanding of the care planning pathway and the differentiation between goals and interventions. 
  • Demonstrate the ability to develop individualized goals utilizing the S.M.A.R.T. acronym. 

This course is approved for 1 Nursing Contact Hour

Continuing Education: This nursing continuing professional development activity was approved by the Virginia Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Criteria for successful completion include completion of the entire course and completion of a post-test with a score of 80% or higher. There are no relevant financial relationships with ineligible companies for those with the ability to control the content of this activity. Nursing contact hours will be awarded for completion of this activity until April 22, 2026.
