This session is intended for all CHAP Home Care accredited agencies or those who wish to learn about the CHAP Standards of Excellence for Home Care. The two-day session will provide an overview of the CHAP standards for Home Care, the most frequent citations in each area, and tips for dealing with these challenges. This service line was previously known as private duty and includes both skilled and non-skilled private duty services.

This event is for all of our customers regardless of what stage of the accreditation process you are in. This is an interactive, virtual event designed to promote the active participation of each individual attendee and, as such, each registration is an individual registration. 

The attendee will be educated on:

  • The format for the Home Care Standards of Excellence
  • An overview of the Standards in each of the 8 Key Performance Areas
  • The top deficiencies in each of the 8 Key Performance Areas
  • Tips for Success to handle the top deficiencies
  • Engagement activities to reinforce knowledge 

This is an interactive and engaging virtual learning opportunity for our CHAP Home Care Accreditation customers or those looking to leverage their home care experience by becoming a CHAP Certified Home Care Consultant!

Participating in this workshop in no way guarantees the outcome of the accreditation process. The content is meant to help participants understand the accreditation process, learn common areas of deficiencies, understand how to gain compliance in the most commonly citied standards and utilize and implement the QAPI process. 

Keri Culhane

Keri comes to CHAP with 17 years’ experience in the post-acute healthcare industry and has held a variety of positions from direct patient care as a Registered Nurse to chief executive in both home health and hospice organizations. She has a passion for community-based care and enjoys educating others and contributing to their growth and development as a healthcare professional. Keri previously served as a board member/officer for the Oregon Hospice & Palliative Care Association. She received her Master’s in Business Administration from the University of Texas at Tyler, and her Nursing degree from the University of Wyoming.

Most recently, Keri served as a Home Care Surveyor for 5 years with The Joint Commission where she conducted surveys for home care organizations across the country and provided education to organizations regarding standards, regulations, and operational processes. During her last year and a half as a surveyor, Keri was responsible for the onboarding and orientation process of new home care surveyors. She has led several projects and workgroups to include educational presentation development, technological survey advancements, standard operating procedure development, and new hire orientation development. In addition to developing educational presentations, Keri has been a speaker/presenter at state association conferences across the country.

Formerly, Keri served as Executive Director for Coastal Home Health & Hospice where she was responsible for overall agency operations and management. She also served as a Director of Accreditation for CHAP responsible for operational oversight and survey activities to validate consistent application of accreditation and regulatory standards for community health providers with a focus on safe, quality patient care delivery.
