HOPE: A Comprehensive Training for Hospice Clinical Leaders and Nurses (CHAP On-Demand)

Welcome to HOPE: A Comprehensive Training for Hospice Clinical Leaders and Nurses. This three-course, comprehensive, asynchronous training is designed to prepare hospice clinical leaders, QA professionals and hospice nurses to accurately complete HOPE assessments, scoring and submission.  

Throughout these courses, learners will explore the implementation of HOPE, required timepoints and assessment timing, HOPE's role in HQRP, HOPE items in sections A through Z, and submission of HOPE assessments.  A case-based scenario will reinforce correct application of guidance, while quality assurance and submission guidance supports compliance and audit readiness.

Upon completion, participants will be able to confidently complete HOPE assessments, reduce errors, support accurate payment and quality outcomes, and align agency practices with CMS expectations. The course is designed for on demand use and serves as both initial training and an ongoing reference tool for hospice clinical managers and nurses
