Exploring Social Determinants of Health for Clinicians

This course shows the clinician how to identify modern social determinants of health as they relate to updated Healthy People 2030 Goals. Realistic examples and scenarios will allow clinicians to learn how to avoid implicit biases, reduce health inequities, and develop individualized, appropriate interventions for all patients. 

Learning Objectives:

  1. Define the terms social determinants of health (SDOH) and healthy inequity.
  2. Identify at least 5 common SDOH encountered in everyday patient practice.
  3. Describe the 5 Healthy People 2030 priority areas related to SDOH and inequity reduction.
  4. Explain how to avoid implicit bias by identifying stereotypes, performing self-reflection, and preventing discrimination based on SDOH.

This course covers the health care challenges and specific health needs of the LGBTQ+ community. Learners will develop their skills in using correct terminology, appropriate assessment and communication practices, and creating an inclusive environment of care. The course provides ample opportunity to practice new skills in the many patient and client scenarios.

This course covers the unique needs of aging LGBTQ+ persons and best practices related to improving access to care, assessing care needs, and planning for care. 

Learning Objectives:

  1. Identify prior life experiences that impact access to health care among aging LGBTQ+ persons.
  2. List three ways clinicians can advocate for aging LGBTQ+ members struggling to access inclusive care.
  3. Identify two risk factors that are often overlooked when accessing the needs of aging LGBTQ+ caregivers.
  4. Distinguish between inclusive and equitable care and marginalizing care in patient care examples.
