Dementia Disease Program Certification

This product contains the standards of excellence for a dementia disease program. Your purchase will allow you to download the standards and access resource tools needed to build a program or to adapt and prepare a current program for your certification review performed by a CHAP Learning Solutions Specialist. Once you are ready for your program survey, you will need to notify us so we can schedule you for your review.
