Welcome to QAPI: Quality Assurance and Performance Improvement for Home Health and Hospice Providers. This course is a comprehensive, asynchronous training designed for home health clinicians, QA professionals, and clinical leaders who are responsible for or participate in their organization’s QAPI program and includes an overview of QAPI, the distinction between Quality Assurance and Performance Improvement, the elements of a QAPI program, QAPI measures and data analysis, building a PIP, and organizational accountability for QAPI.

Upon completion, participants will have increased knowledge of the Centers for Medicare & Medicaid Services (CMS) QAPI requirements for home health and hospice organizations. 

• Define QAPI (Quality Assurance Performance Improvement) and explain its purpose in home health and hospice.
• Identify core CMS (Centers for Medicare & Medicaid Services) expectations for an ongoing, organization-wide, data-driven QAPI program.
• Differentiate quality assurance activities from performance improvement activities.
• Select meaningful quality indicators and data sources for monitoring performance.
• Describe the steps for designing, implementing, and sustaining a performance improvement project (PIP).
• Explain staff and leadership roles in creating a culture of quality and safety.

Developing a care plan that is individualized, compliant, and effective is a crucial component of successfully delivering care that is patient-centered with improved patient satisfaction and outcomes, whether those be curative or palliative in nature. This course will address what the Conditions of Participation and CHAP standards say about care planning, and the process for developing a patient-specific care plan, including a special focus on developing goals that are individualized, specific, and measurable.

Charting The Course: A Guide to Effective Care Planning in Home Health & Hospice

Welcome to Advanced Home Health Value-Based Purchasing (HHVBP).  This course is designed to expand your understanding of the HHVBP Model including how to read and interpret your quarterly and annual reports to help you create an action plan that will drive quality measures to improve patient care and mitigate payment reductions.  

After completion of this course, learners will have the ability to:

• State the measure categories and associated payers for each. 
• Define a HHVBP Baseline Year and how it relates to the Performance Year in HHVBP.
• Identify the Small and Large Cohorts, and how the measurements for each compare and contrast relative to minimum data requirements.
• Recognize how the new measure MSPB-PAC (Medicare Spending Per Beneficiary/Post Acute Care) is calculated.
• Analyze how to read and define your Performance Reports: Interim and annual.

This on-demand, asynchronous course is designed to educate home health providers about the Program for Evaluating Payment Patterns Electronic Report (PEPPER), why understanding PEPPER matters and how to access and interpret your agency's PEPPER data and act upon the data to achieve compliance.

• Explain what a Home Health PEPPER is and why CMS produces it. 
• Identify the claims and episodes of service that PEPPER summarizes. 
• Describe how target areas are calculated and what the 80th percentile means.
