CHAP Infusion Pharmacy Standards of Excellence v.1.0.0

In keeping with its goal of elevating the quality of all community health care, the Community Health Accreditation Partner (CHAP) continually reviews and revises the “Standards of Excellence” to ensure currency and relevance to the community health care industry. The Infusion Pharmacy Standards were developed to address the scope, complexity, and challenges of providing Infusion Pharmacy services in a variety of community-based settings. Compliance is ensured with applicable key performance areas from the CHAP Pharmacy Standards of Excellence and the DMEPOS Standards of Excellence.

The Pharmacy Standards were designed to:

• Promote ease of application, interpretation and use of standards
• Emphasize the importance of the interests and rights of Pharmacy clients
• Strengthen the long-term viability of Pharmacy organizations
• Advance the recognition of the Pharmacy organization as an integral component of the national health care delivery system
