Note: Version 4.2.0 incorporates updates to align with the "Revisions to Home Health Agencies (HHA) - Appendix B of the State Operations Manual (QSO-24-07-HHA)". Additionally, this version introduces a new Key Performance Area focused on Compliance Programs.

The CHAP Standards of Excellence were effective January 13, 2018 with the most recent revision occurring August 1, 2024.

These standards also apply to Medicaid home care providers seeking initial or renewed accreditation in states that require compliance with the Medicare home health CoPs.
