CHAP Community Care Facility Certification Standards of Excellence v1.0.0

Community Care Certification is the organized delivery of resident1 and family/caregiver-centered care that optimizes safety and quality of life by assessing and anticipating resident or resident needs, preventing injury, and treating symptoms as they occur. Resident safety and quality of life are measures and outcomes addressed under the Performance Improvement Chapter requirements.

The Community Care Facility standards address the physical, emotional, social, and spiritual needs of the resident and the family, as well as facilitating access to information and choice. It is provided and coordinated by a team of various providers.
