Aide care plans are essential tools for translating the interdisciplinary plan of care into clear, patient-specific instructions that guide safe, consistent, and person-centered care. In this one-hour, on-demand course, home health and hospice professionals will explore how to develop, implement, supervise, document, and update aide care plans that reflect each patient’s assessed needs, preferences, functional abilities, risks, and goals of care. Learners will examine the distinct responsibilities of clinicians, aides, supervisors, and interdisciplinary team members; identify the essential components of an individualized aide care plan; and apply practical strategies for maintaining appropriate role boundaries and ensuring that assigned tasks remain aligned with the current plan of care. Through realistic home health and hospice examples, interactive scenarios, knowledge checks, and a mini-audit activity, participants will practice distinguishing clear, measurable instructions from vague or incomplete directions, recognizing findings that require prompt communication, and selecting appropriate actions when a patient’s condition or care needs change. The course also addresses accurate visit documentation, aide supervision, competency validation, common care-plan deficiencies, and the use of quality findings to strengthen care planning. By the end of the activity, learners will be better prepared to support compliance, continuity, patient safety, dignity, and effective interdisciplinary communication across home health and hospice settings.
Target audience: Home health and hospice registered nurses, licensed practical/vocational nurses, home health aides, hospice aides, clinical supervisors, educators, quality professionals, and agency leaders.
