Age-Friendly Care -Certified Professional (AFC-CP) Course

This course is designed for care providers seeking to enhance understanding and apply age-friendly care principles. Participants will learn to align the care they provide with the 4Ms framework—What Matters, Medication, Mentation, and Mobility—ensuring that older adults receive personalized, evidence-based care.

This course equips care providers with the knowledge and skills necessary to deliver high-quality, age-friendly care within their professional scope of practice. By the end of the course, learners will be able to apply age-friendly care principles to improve patient outcomes, promote overall wellness, and support cost-effective care solutions.

The course is intended for a broad range of care professionals, including but not limited to Home Health and Hospice Aides, Social Workers, Chaplains, Physical Therapists, Occupational Therapists, Nurses, and other individuals who interact with patients or families, such as office staff and administrators.

Upon successful completion of the course and passing the certification exam, participants will earn the CHAP credential, Age-Friendly Care – Certified Professional (AFC-CP). This certification is valid for three years, after which renewal is required.

After completion of this course, learners will have the ability to:

2. Identify needs, risks, and considerations when working with older adults and their families.
