This ON DEMAND webinar will provide Part 2 of the information independent providers need regarding the NEW CHAP Palliative Care Certification and the process to becoming certified. 

Palliative Care is the organized delivery of patient and family-centered care that optimizes quality of life by anticipating, preventing and treating symptoms of individuals with serious illness. Palliative care addresses the physical, emotional, social, and spiritual needs of the patient and the family, as well as facilitating access to information and choice. It is provided and coordinated by an interdisciplinary team in any setting.

Topics will include how to meet certification standards for delivery of palliative patient care and how to speak and educate others about your palliative care program.

Barbara McCann

CHAP President and CEO

Barbara McCann is CHAPs President and CEO. She previously served as the Chief Industry Officer of Interim HealthCare Inc., the nation’s largest home care and hospice franchisor with over 300 offices in 44 states.  Ms. McCann joined Interim HealthCare in January 1998, and served as the Chief Clinical Officer overseeing the company’s corporate clinical operations team developing policies, procedures, and practice guidelines for the delivery of patient care, as well as compliance with federal law and regulation and professional standards of practice.  She also directed the national chronic care programs.  In 2010, Ms. McCann assumed the role of Chief Industry Officer acting as the company liaison and industry representative to Congress, CMS, MedPAC, the National Quality Forum, and the Patient-Centered Primary Care Collaboration, representing the roles of home care and hospice in medical homes and ACOs, as well other professional and consumer organizations.  In addition, Ms. McCann directs the national care transition programs partnering with hospitals, physicians, and payors to reduce avoidable readmissions and effectively transition chronically ill patients and their families to home self-management and safety. 

Ms. McCann is a Phi Beta Kappa graduate of the University of California at Berkeley where she received her Bachelor’s degree in Social Welfare.  She received her Master’s degree from the University of New Mexico, and actively practiced as a medical social worker in a variety of health settings, including home care and hospice, prior to joining the JCAHO in 1981.

Jacqueline Lindsay, BSN, RN, MPH, JD

Director of Accreditation, CHAP

Jacqueline L. Lindsay is a professional Registered Nurse who holds a Master of Public Health from New York University and a Juris Doctorate degree from Hofstra University School of Law in Hempstead, New York. Jacqueline began her career in community-based care with the Visiting Nurse Service of New York, as a home care nurse providing skilled nursing care to homebound elderly patients.

For the past 30 years she has held management and administrative roles at both for-profit and non-profit home health organizations where her passion and commitment to quality patient care has been well-recognized. Her published works include “Improvement in Medication Management" in the home setting.

Jacqueline joined Community Health Accreditation Partner (CHAP) as a Site Visitor in March 2012. She currently serves as Director of Accreditation at CHAP's Arlington, VA headquarters.
