This ON-DEMAND education series provides 14 self paced courses, a discussion group forum and focuses on the Medicare Home Health conditions of participation (CoPs) effective January 13, 2018.  

Learning Objectives:

At the end of this series, participants will be able to:

  1. Name the benefits and responsibilities of a CHAP Certified Home Health Consultant.
  2. Outline the CHAP Accreditation process and how best to educate clients on the process. 
  3. Identify the modifications and additions within the home health Conditions of Participation.
  4. Identify trends in deficient practice under the home health Conditions of Participation. 
  5. Assist clients in applying performance improvement strategies specific to identified deficient practices.

Completion of the entire series and the evaluation is required to obtain contact hours. 

This education activity will award 13.75 nursing contact hours for successful completion until October 18, 2020.

    Bobbie Warner BSN, RN

    Director of Accreditation, CHAP

    Bobbie Warner is a registered nurse and has worked in community health for over 25 years in multiple capacities. Bobbie began her community based career by providing direct patient care for several years in home health, hospice and private duty. Management roles included responsibility for the startup of a private duty agency whose focus was provision of care to  ventilator dependent children and then as  Director of Quality Assurance and Education for a large community health organization. After serving in these roles, Bobbie joined CHAP in 2009 as a site visitor who enjoyed the additional roles of preceptor and Team Leader. In 2014 she moved into her current role as Director of Accreditation. Bobbie obtained her BSN from Eastern University.


    Fran Petrella, BSN, RN

    Senior Vice President, Accreditation

    Fran Petrella is the Vice President of Accreditation at Community Health Accreditation Partner (CHAP) in Washington, DC. CHAP was the first accrediting body for community-based health care organizations in the United States and was created in 1965 as a joint venture between the American Public Health Association (APHA) and the National League for Nursing (NLN).

    Fran graduated Cum Laude from Washington State University in 1977 with a Bachelor's of Science in Nursing. Fran began her career as a nurse's aide in a nursing home in 1971, moving to the hospital on graduation, and working on an Internal Medicine/Oncology unit. In 1986, she found home care, providing both home health and hospice services as a field nurse and in management in northwest Washington State. In 2000, she started working as technical support staff and Home Health expert resource for a home care benchmarking company, advancing to Director of Client Services within 6 months. Fran jumped back into the Home Care arena in 2010, working in management/oversight roles for a VNA, and facilitating the transition of a merger with a hospital-based system, providing home health, hospice, and home infusion pharmacy services. In 2012, she moved across the country from Washington State to join the team at CHAP. Community-based care is her heart. She strongly believes that quality community-based care should be at the core of health care and that the increased focus towards “connecting the dots" through the continuum of care is helping us along the path to that change. Her ultimate goal is to be involved in the path in whatever way she can; to facilitate bringing quality home care closer to becoming the center of the path along the continuum.

    Jacqueline Lindsay, BSN, RN, MPH, JD

    Director of Accreditation, CHAP

    Jacqueline L. Lindsay is a professional Registered Nurse who holds a Master of Public Health from New York University and a Juris Doctorate degree from Hofstra University School of Law in Hempstead, New York. Jacqueline began her career in community-based care with the Visiting Nurse Service of New York, as a home care nurse providing skilled nursing care to homebound elderly patients.

    For the past 30 years she has held management and administrative roles at both for-profit and non-profit home health organizations where her passion and commitment to quality patient care has been well-recognized. Her published works include “Improvement in Medication Management" in the home setting.

    Jacqueline joined Community Health Accreditation Partner (CHAP) as a Site Visitor in March 2012. She currently serves as Director of Accreditation at CHAP's Washington DC headquarters.

    ​Jacqueline King, MBA, MHA, RRT

    Director of Accreditation, CHAP

    Jacqueline King, MBA, MHA, RRT, is a Registered Respiratory Therapist who has been working in Healthcare for 28 years. She joined CHAP as Director of Accreditation in 2011 and previously held management positions in various healthcare environments, including Acute Care, Sub acute Care, Specialty Hospitals, and various Community Based Home Health businesses (SNF and HME). Jackie earned her BS at Long Island University in Brooklyn NY, and two Masters Degrees from the University of Maryland University College (MBA, MHA). Jackie enjoys working with an organization focused on improving the delivery of healthcare, and building quality organizations.

    Patricia Gauvin-Gaskell, BSN, MBA, RN

    Senior Accreditation Manager

    Patricia Gauvin-Gaskell is the Senior Accreditation Manager at CHAP and was previously a CHAP Site Visitor.


    Belinda Bokuniewicz RN BSN COS -C

    Senior Accreditation Manager, CHAP

    Belinda Bokuniewicz is a Senior Accreditation Manager and previously served CHAP in the site visitor role when first joining the organization in 2015.  She has 20 years experience working in the post-acute continuum of care having held various roles in leadership and administration, marketing and consulting.  Belinda is very passionate about high quality person centered care and enjoys working with organizations nationwide.  She is currently pursuing a Master of Science in Health Administration at the University of MI - Ann Arbor.

    Jennifer Shepherd, MSN, MHA, RN-BC, NEA-BC, CHPN, CCRN-K

    Director, Educational Services, CHAP

    Jennifer Shepherd has 25 years’ experience in healthcare and nursing leadership. She is best known for her clinical management and professional development capabilities; leading teams to cultivate and implement innovative education strategies, ensuring achievement of organizational goals. She has master’s degrees in both nursing and in healthcare administration and is currently obtaining her DNP.  Jennifer joined CHAP as Director of Educational Services in 2018, serves on the Virginia Nurses Association Board as Commissioner of Workforce Issues and holds a position on Capella’s School of Nursing and Health Sciences Advisory Board. She holds advanced certifications as a nurse executive, in hospice and palliative nursing, pediatric critical care and nursing professional development. 

    Lisa Goodlow, BSN, RN, MSM

    Director of Accreditation, CHAP

    Lisa Goodlow, BSN, RN, MSM is Director of Accreditation at Community Health Accreditation Partners.  Prior to joining CHAP, Lisa served as the executive director of home care services for a non-profit agency in Ohio.  Lisa brings more than 30 years of experience in home care, hospice and private duty services to her role.  She is a bachelor’s prepared Registered Nurse and holds a master’s degree in management.  She also previously served on the Ohio Council for Home Care Board of Directors.
